Details:
After completing Back to the Head Inquisitor - 8715, Inquisitor Velman offered me several quests that were well above my level (the next level "Further Training" quest, for example; I am only level 2 and just did further training). When I cancelled out and got back in, he only offered me the quests he was supposed to.

Seems to be working fine currently. Could have been a random hiccup with the server. Perhaps a Content Designer was reloading quest scripts or spawn scripts at the time.
