Details:
During "A Second Chance" quest on Isle of Dawn, I clicked the chest near the altar to get the ember shard and despite it telling me that I looted it, it didn't add the shard to my bags. The chest looks to be clickable, but I can't interact with it anymore. If it makes any difference, my "general inventory" was full at the time when I tried to loot, but my smaller bags were completely empty.

If this player has an ammo case, there's a very good chance it went in there.
