Details:
Eye of the Beholder - 3794 criteria met doesn't complete (quest giver still blue) quest is kill 2 of each type of Kojuko, after all 6 are killed quest giver doesn't change to green and accept/give rewards.

Quest giver and quest turn in are two different NPCs. Turn in is with Master Kaol, Ghost Wind Instructor. He's in the camp south of the questgiver, on the eastern edge of the bay. Quest text mentions this: "my pupil, Master Kaol, will need to further you along." and "On your way to Kaol, clear the Kojuko from the path"
